Male Infertility


heraldsun.com.au - You fall in love, you get married, you have kids… or so the story goes. Sadly, the statistics prove otherwise: one in eight couples in Australia will have difficulty conceiving, and be classified "infertile".

And while infertility usually falls into the category of "secret women's business" and is often perceived as a female problem, it is estimated that in Australia, infertility affects about one in every 20 men.

For half of all infertile couples, the problem lies with the male partner, while in 40 per cent of infertile couples using assisted reproduction technologies, the underlying reason is male infertility.

But rather than being exclusively a female or male problem, infertility is a couple’s problem, says Professor Robert McLachlan, director of Andrology Australia, which specialises in male reproductive health and medicine.

"It takes two to be infertile," he says. "For various reasons, when couples can’t conceive, it is a tremendous blow to both the individual and the couple. This is why we say it is a couple’s issue, as medically, we treat the couple."

However, because of the misconception that infertility is a female condition, many men are completely unprepared for such a diagnosis and are often left emotionally battered when they discover that the reproductive problems are theirs.

"For a man who is really looking forward to being a dad, it's absolutely devastating," Professor McLachlan says. "A diagnosis can cause a great deal of emotional pain and affect a man’s self-esteem. Most infertile men struggle with the idea that they are not able to do what other men can do. This can lead men to confuse their infertility with their sense of masculinity, sexuality, virility and potency."

While infertility can be devastating to a couple, the good news is that, due to the advancements in fertility treatments and the understanding of male infertility and sperm function, it is now possible for many infertile men to become fathers.

Cause and effect

So what is male infertility? Simply defined, it is when a man cannot ejaculate semen that contains sufficient numbers of normal, functional sperm to pass through the female tracts and fertilise the egg.

About three-quarters of all male infertility problems are due to a malfunction in the sperm production process, with low numbers being produced or sperm motility problems (when the sperm don’t swim properly).

In a minority of men, there is a recognisable cause for this malfunction, such as hormonal problems or a past history of undescended testes, mumps or cancer treatments. But usually, there is no cause identified and a problem with the genetic control of sperm production is suspected.

Second on the list, and affecting one in four infertile men, are blockages that lead sperm away from the testes to the penis, which causes a complete lack of sperm in the ejaculated semen.

One of the greatest steps forward is intracytoplasmic sperm injection (ICSI), a form of in vitro fertilisation (IVF) where a single sperm is injected directly into the egg by piercing the outer covering of the egg. This method has been practised in Australia since 1993.

Professor McLachlan says it is helpful for two-thirds of all infertile males. "The ICSI procedure has changed the landscape of male infertility,” he says. “It has meant that there are many more men for whom there is a good possibility of fathering their own genetic children."

A common genetic abnormality leading to male infertility is Klinefelter’s syndrome, where the man has an extra X chromosome. This condition affects one in 600 men.

"The vast majority of these patients do not have sperm in the semen, have low testosterone levels and very small testicles, about the size of grapes," says Professor McLachlan.

"Interestingly, these guys can be more prone to childhood learning difficulties and behavioural problems. However, it is when they present with infertility problems that it is first discovered that they have this syndrome, which may explain some of their past problems, such as breast development in puberty. We prescribe them testosterone, which aids physical development. For a third of these men, we can harvest testicle tissues in the wall of the testicles with ICSI and they go on to have healthy children with a normal chromosome count."

Other options

But for some infertile men the diagnosis is not as positive. "For some, male infertility is completely untreatable, due to the man having no sperm in his semen or testes, or no sperm-producing cells in the testes, because they either did not develop or have been permanently destroyed,” Professor McLachlan says.

"The only options for these couples is to consider adoption or donor insemination. Sperm donors provide an extraordinary gift by giving life and happiness to another family."

Is there even such a thing has having too much sex? I know the men out there reading this are going to disagree with me on this but there actually is such a thing as being oversexed. The idea of too much sex is a relative term as some couples can easily go through 3, 4 or even 5 times a day or more of sexual activity, while other couples might not engage in sex for months. You might be asking yourself, what does this matter to anything.

To me the key to good sexual health is the steady activity of sex consistent in every males life. Now the only way to do this is if you are on “good” terms with your partner. If they feel that they are just being used like a piece of meat, they are oversexed and will end up shutting you out.

While doing this, it will lead to a period of you being frustrated and undersexed and cause problems with your own sexual health.

Do you see what I am getting at here? By taking care of your partner and their sexual appetite level you are going to ensure that you continue to have a consistent amount of sex. If you try to force it upon them, you will end up alienating yourself and end up sleeping on the couch more often then you like.

Another important aspect of sexual health is confidence. If a man is not confident, he will not perform well. While the importance of the size of a penis to women highly debated, it’s hard to argue that it is beneficial to men themselves.

For an example, look no further than this Brazilian gentleman who has given himself breast, shoulder, and trapezius implants then decided to upload a video of himself dancing around to YouTube.

This is a pretty big reminder to everyone out there that yes, we men are also susceptible to having body image issues. It’s also an even bigger reminder that if you shove an orange or five under your skin, you can reasonably simulate the look of muscles without all that difficult “working out” or “lifting heavy objects.”

I can certainly relate to wanting to look good. But there is not a woman on Earth whom I could see myself getting implants for. Unless it was for a hydraulic penis. Because then, not only do I have a magical thrusting set of genitals, no longer would I need to keep a jack in the back of my car. It just makes economic sense.

Alcohol May Reduce Men's Heart Risk 51%


Study Shows Moderate Drinking Cuts Risk of Heart Disease in Men by 51%

Nov. 18, 2009 -- Regular consumption of alcohol -- beer, wine, or hard liquor -- reduces the risk of heart disease in men by a third or more, according to a new Spanish study.

''Our study confirms what many other studies have already said," says researcher Larraitz Arriola, MD, of the Public Health Department of Gipuzkoa in San Sebastian, Spain. One difference, she says: Researchers in the new study separated ex-drinkers from lifelong teetotalers in hopes of better understanding the alcohol- heart health link.

Arriola and colleagues also found a beneficial effect of alcohol for women's heart health, she says, but it was not strong enough to be considered statistically significant. She suspects it's because of the relatively low number of women in the study who developed heart disease.

While drinking was associated with heart health, Arriola is quick to offer this caveat: ''I would not advise anybody to [start to] drink alcohol, because alcohol causes, as we mention in our paper, 1.8 million deaths a year" in addition to disabilities.

"If somebody already drinks alcohol, then I would advise to drink moderately, eat healthy food, and do some exercise."

In the study, researchers evaluated more than 41,000 men and women enrolled in the ongoing European Prospective Investigation into Cancer (EPIC) study. That study includes a half million adults living in 10 Western European countries.

In the current research looking at alcohol and heart health, the researchers evaluated 15,630 men and 25,808 women ages 29 to 69, all free of heart disease at the beginning of the study, following them for a median of 10 years (half longer, half less).

The researchers calculated alcohol intake from a diet history record; a follow-up revealed which participants had a cardiovascular event -- either a heart attack or unstable angina (chest pains) that required a procedure such as a bypass operation or angioplasty.

During the follow-up, 609 such events occurred to 481 men and 128 women.

Spain has low heart disease death rates in comparison to some other countries, but high levels of alcohol consumption.
Amount of Alcohol and Heart Risk

Drinking any type of alcohol lowered the risk of serious heart disease in men, with the amount of risk reduction associated with the amount of alcohol:

* Light drinking reduced risk by 35%
* Moderate drinking reduced risk by 51%
* High and very high levels of drinking reduced risk by 54% and 50%.

Former drinkers had a 10% risk reduction.

For the study, the researchers considered a drink as an alcoholic beverage with 10 grams of alcohol, the U.K. standard, Arriola says. In the U.S., a standard drink is equal to 13.7 grams of alcohol, according to the CDC.

Roughly, here is how Arriola defines her categories:

* Light drinking was up to 5 grams a day -- or about one glass of wine, one and one-half beers, or less than a half glass of hard liquor.
* Moderate drinking was 5 to 30 grams a day, or about two glasses of wine, two or three beers, or a half to one glass of hard liquor.
* High and very high levels of drinking were 30 to 90 grams a day, or about five or more glasses of wine, seven or more beers, and one to one and a half glasses or more of hard liquor.


While the type of alcoholic beverage consumed, overall, did not have an effect on the level of risk reduction, the researchers found the protection greater for those drinking moderate to high levels of alcohol, which included beverages other than wine alone.

The study results replicate many other studies, according to Kristi Reynolds, PhD, MPH, a research scientist and epidemiologist at the Kaiser Permanente Southern California Medical Group. But she points out that heavy alcohol consumption carries many risks.

In an email, she writes that heavy alcohol consumption has been shown in other studies to lead to increased illness and death from other causes. "Therefore, the implications of these findings should be examined cautiously. Advice regarding alcohol consumption should be tailored to the individual patient's risks and the potential benefit."

The researchers, as part of a larger studied, looked at the link between low levels of vitamin D and heart disease risk in a subset of men taking part in a cancer study who are also a part of an ongoing national health survey that includes women and men. The study was designed to compare disease risk among people with low levels of vitamin D to those with higher levels.

Free chemical forms of testosterone and estrogen in the men were measured for the study. No link was found between low testosterone levels, vitamin D, heart disease and bone health. However, lower estrogen levels, combined with low vitamin D levels revealed increased risk for heart disease and osteoporosis in the men.

"These results reinforce the message of how important proper quantities of vitamin D are to good bone health, and that a man's risk of developing osteoporosis and heart disease is heavily weighted on the complex and combined interaction of how any such vitamin deficits interact with both their sex hormones, in particular, estrogen," Michos says.

The researchers plan to study blood samples in women to see if the findings are the same. The new research supports past studies that low levels of vitamin D increase an individual’s risk for heart disease. According to Dr. Michos, a source of confusion exists because estrogen replacement in women does not reduce, and may even increase heart disease risk.

In the meantime, studies are ongoing to clearly define the role of vitamin D for preventing heart disease, stroke and for keeping bones healthy. For now, Dr. Michos recommends boosting vitamin D intake by consuming cod, sardines, and fortified dairy products, in addition to vitamin supplements. She also recommends briefly exposing skin to UV rays during warmer weather to boost vitamin D levels.

Arnold Schwarzenegger Signs Donda West Plastic Surgery Law

It's been a busy week for California Governor Arnold Schwarzenegger. In addition to signing a strong new anti-paparazzi bill, the governor signed a bill, called the Donda West Law, that requires anyone undergoing plastic surgery in the state to receive a mandatory physical examination first, according to the Los Angeles Times.
 
The bill was written in response to the death in 2007 of Kanye West's mother, Donda, 58, who died of complications related to cosmetic surgery. Dr. West's family reportedly believe that a pre-op physical exam would have uncovered coronary artery disease. West underwent several plastic surgery procedures without medical clearance and her coronary disease and postoperative factors are believed to have contributed to her death.


The California state Senate passed the measure last summer, with California being home to the largest plastic surgery industry in the U.S. Dr. West's niece, Yolanda Anderson, was one of the primary movers behind getting the initial bill passed, which also requires a mandatory written clearance before surgery. However, facing a flood of bills on his desk at the time, Schwarzenegger vetoed the first pass of the bill in February 2008, then signed it after it was resubmitted this past February.


"Sometimes patients may think they are well enough for cosmetic surgery, but in reality are not," said Assemblywoman Wilmer Amina Carter, one of the leading sponsors of the bill. "This bill will potentially save lives."
After his infamous run-in with Taylor Swift at the VMAs last month, Kanye West told talk-show host Jay Leno that his mother would have been disappointed in his behavior. Genital damage could be reversed after tissue is grown in laboratory

Damaged human genitals could eventually be restored to full function after penis tissue has been successfully grown in the laboratory and implanted into animals.

Scientists in the United States have replaced the erectile tissue of rabbits with a cultured version grown from cells in a dish, restoring normal sexual function and allowing the animals to father offspring.

The research is the most successful attempt yet to engineer penis tissue that works normally when implanted, and promises to have benefits for reconstructive surgery for human patients.

At present there are few options for men who suffer penis damage because of injury or cancer, or who are born with genital abnormalities. While the penis can sometimes be rebuilt for cosmetic effect, it is rarely possible to restore sexual function because erectile tissue cannot be replaced.

The research suggests that it should, ultimately, be possible to grow new erectile tissue using cells from the penis seeded on to a shaped 3-D lattice.

This could also be used to help men with certain forms of impotence caused by tissue damage. Another potential application is in cosmetic surgery for penis enlargement, though this will be limited by the risk of damage to a healthy organ.

The procedure is unlikely to be useful for female-to-male transsexuals, however, because the erectile tissue has been grown from penis cells. Stem-cell methods could provide a solution but further research would be needed.

Anthony Atala, director of the Wake Forest Institute for Regenerative Medicine in North Carolina, who led the research, said: “Further studies are required, of course, but our results are encouraging and suggest that the technology has considerable potential for patients who need penile reconstruction.

“Our hope is that patients with congenital abnormalities, penile cancer, traumatic injury and some cases of erectile dysfunction will benefit from this technology in the future.”

The success of reconstructive surgery for damaged male genitals has been limited by the complex structure of the mammalian penis. Erections rely on a pair of chambers of sponge-like tissue, known as the corpora cavernosa penis (cave-like bodies of the penis), which fill with blood on sexual arousal to make the organ stiff.

These can be lost because of surgery for penile cancer or traumatic injury, or they can be small or absent because of congenital abnormalities. The corpora cavernosa can also atrophy in some forms of impotence, for example among prostate cancer patients who have no erections for a long time after surgery.

Reconstructive techniques generally rely on rebuilding the penis using skin and tissue from the arm or leg and rolled up around the urethra. This can then be attached to nerves to provide some sexual sensation, and stiffened for intercourse by inserting a silicone prosthesis.

In the research, which was published in the journal, Proceedings of the National Academy of Sciences, Dr Atala’s team first removed smooth muscle cells and endothelial cells, which line blood vessels, from the erectile tissue of rabbit penises.

These were then injected into a 3-D scaffold and cultured so that they multiplied along it. The tissue-covered scaffold was then implanted back into the rabbits’ penises, from which erectile tissue had been excised.

The results were an improvement on previous experiments, in which the Wake Forest team had managed to create erectile tissue with about 50 per cent of normal function. A new technique was used to seed the scaffold with smooth muscle cells twice, allowing it to hold six times as many as was previously possible.

The extra smooth muscle cells seem to be important to restoring sexual function because they play an important role in erections. The muscle cells relax during arousal, triggered by the release of nitric oxide from endothelial cells, allowing blood to engorge the penis.

“Increasing the density of smooth muscle cells led to normal erectile pressures within the tissue,” Dr Atala said. “These results are encouraging. They indicate the possibility of using laboratory-engineered tissue in men who require reconstructive procedures. A lack of erectile tissue currently prevents us from restoring sexual function to these patients.”

In July, Richard Curry reported on the Vietnam Veterans of America website that “These vets were developing diabetes, Hodgkin’s disease, and more than 30 varieties of cancer, yet they remained unaware that their illnesses were related to military service, and that treatment as well as disability benefits were available to them through the VA.” VHC (http://www.veteranshealth.org) is working to “ensure that veterans get proper service-related diagnoses in a timely manner, and are fully appraised of their options for care and compensation.”

Bladder cancer is the newest addition to a list of health risks facing military veterans. National Institute for Health reports the prevalence rate of bladder cancer among veterans is increasing. Bladder cancer has long been recognized as a leading occupational cancer because of workplace exposure to toxins. Giora Katz, M.D, urologist, reported in 2006, “A multi-center study of a NMP22 point-of-care test for bladder cancer diagnosis found the veterans’ hospitals represented 9% of the sites in the study, but accounted for 16% of the cancers. All patients presented with the same symptoms (hematuria – blood in the urine, or painful or frequent urination), but the percentage of those diagnosed with bladder cancer at the veterans’ hospitals was 11.1%, compared to 5.7% at the community and academic practices. In addition, most of the malignancies at VA hospitals were already in an early invasive stage, compared to the other sites where the most common stage was noninvasive.”

Bladder Cancer is often referred to as one of the costliest cancers in part due to the high recurrence rate of this cancer. Today, more than a half million people have had or are living with bladder cancer. According to government data, bladder cancer annual costs are almost $3 billion compared to $1.5 billion spent on prostate cancer. Consider these bladder cancer facts: Smoking is the major risk factor followed by workplace exposure to chemicals and combustible gases. Other risk factors include, atomic site exposure, arsenic in drinking water, chemotherapy or radiation, family history, as well as, a medical history of gonorrhea. Veterans as a group have a higher incidence of smoking; an estimated 40 percent of veterans ages 65 year and older are smokers.

Death, Illness Among Men Examined

Unlike football, food and family life, preventative health is not something you hear men talking about often. However, now may be a good time to start a discussion. Here's why:

The statistics regarding men's health are grim. Men die at a younger age than women from the top causes of death: heart disease, cancer, stroke, chronic lower respiratory disease and accidents. On average, women live five years longer than men.

If more men practiced preventative health measures including lifestyle changes and increased doctor visits, approximately 60 percent of chronic diseases among men, as well as most injuries and accidents, could be avoided.

So listen up, gentlemen! Here are some immediate steps you can take to improve the quality and length of your life:

* Eat a diet rich in fruits, vegetables, whole grains and low-fat foods.
* Be careful to limit cholesterol intake and avoid saturated fats.
* Try to get enough exercise. Moderate exercise for 30 minutes five times a week, or vigorous exercise for 20 minutes three times a week should do the trick.
* Protect yourself from the sun to lower your risk of getting skin cancer.
* Maintain a healthy weight. Think lean. Normal is OK, but you actually want to be on the lower end of the normal range of the body mass index -- a number that relates your weight to your height. Aim for a BMI of 21 to 23 percent -- for a man 6-feet tall, that's about 162 pounds.
* Stay hydrated. Drink about eight, 8 oz. glasses of water per day.
* Don't drink any more than two alcoholic drinks per day.
* Don't smoke, and limit your exposure to second-hand smoke as much as possible.
* See your doctor for regular check-ups.
* Discuss your family health history with your doctor in as much detail as possible.
* If you are over 40, get a baseline PSA (prostate specific antigen) test and monitor this periodically with your doctor.
* Practice safe sex.
* Wear a seatbelt whenever you are in a car, and wear a helmet when you ride a bicycle or motorcycle.
* Manage your stress.
* Ask for help if you need it.
